Dementia Support Services available to Wirral Patients and their carers.             

The all age mental health crisis line can be contacted 24/7 on 0800 145 6485.                                                                                                                                                                                                                                    

CWP Wirral Memory Assessment Service – assessment/diagnosis for people who have suspected or diagnosed cognitive impairment – 0300 303 3157.

CWP Adult Cognitive Assessment function  (under 65s) – ACAS is a multi-disciplinary team who work with people who are under 65s. who have cognitive impairment – 0300 303 3157.

Community Trust Nurse Practitioner for Older People (NPOPs)– full holistic assessments and support for people living with dementia and carers, including referrals to other support services – 0151 514 2222 or wcnt.gpreferrals@nhs.net.

Accepts self-referrals (professionals can also refer)

Age UK Wirral

Day Care Service – Two units for people over 65. Devonshire Resource Centre and Meadowcroft Community Wellbeing Hub – 07514 990 684 or 0151 482 3456 for more information.

Bradbury Wing – specialist day service for people under the age of 65 – 0151 482 3456.

Driving Assessments – a clinically led assessment process including evaluation of physical, cognitive and vision skills for driving – 01745 584858 or mobilityinfo@btconnect.com.

Reminder Finders – a 12 week programme of MCST (Maintenance Cognitive Stimulation Therapy) aimed at clients who have recently received a diagnosis of dementia. Held at the Devonshire Resource Centre – 0151 488 7804 or lisa.cummings@ageukwirral.org.uk.

Carers support – emotional and practical support and advice available to carers of a person living with dementia – 0151 482 3456.

Alzheimer’s Society

Alzheimer’s Society online & telephone support – (0300 150 3456) Referrals will be passed to community based dementia advisor for more complex cases who will provide face to face support, advice and information and signpost to other partners as appropriate.

Dementia Together Wirral

Welcome Me – Retain Wellbeing

Wired

support for carers including information, advice, dementia carer support (emotional & practical advice), emergency contact service and training – 0151 670 0777.

Wirral Falls Prevention Service

for clients living within the community to be eligible for our service, they must have capacity, capability and willingness to independently follow advice and particularly exercise programmes – 0151 541 7709 or fallspreventionsecure@medequip-uk.com.

Social Service Central Advice and Duty Team (CADT) – provide support and advice to people experiencing difficulties managing dementia. Call 0151 514 2222 (option 3). If you are calling after 5pm and your enquiry is urgent, please contact the out of hours Emergency Duty Team on 0151 677 6557.

National Dementia Helpline – Freephone number open 7 days a week, providing instant access to advice and information for anyone affected by dementia – 0333 150 3456.

Talking Point – Our Online Community – 24 hour online support and discussion for anyone affected by dementia. You can ask questions and get practical advice and emotional support from other people who have shared experience.

Dementia UK – a national Admiral Nurse helpline which is 0800 888 6678. They can also be contacted on helpline@dementiauk.org – visit dementiauk.org/book to book a phone or video appointment with an Admiral Nurse in our virtual clinic.

Drop ins

NPOPS run a drop in at Marine Place (within Marine Lake Medical Practice) every Wednesday 2pm – 4pm no booking required.

Alzheimer’s Society hold a drop in every other Tuesday at Marine Place (within Marine Lake Medical Practice) 10am-12pm with a dementia advisor.
