New Patients

If you live within our practice area (Moreton and some areas of Hoylake, Upton and Greasby), you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

Online Registration

In-person Registration

Please collect a registration pack from the surgery. Once you have completed these documents, please sign all forms and bring these to the surgery along with proof of your current address and photographic identification for each person registering. If you are unable to do this or have any queries, please contact us on 0151 677 2425. You do not need proof of ID, address, immigration status or an NHS number in order to receive care or see a GP.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
